Help Shape the Future of the Epilepsy Foundation of Greater Chicago

As a member of our community, whether you’re living with epilepsy, caring for someone who is, or supporting EFGC through donations and advocacy, we invite you to share your voice in our 2025 Community Survey.

This anonymous survey is an opportunity to provide feedback on your experiences with the Epilepsy Foundation of Greater Chicago. Whether you’ve attended our events, used our services, or supported our mission in other ways, your input will help us better understand the needs of our community and guide our efforts as we look to the future.

The survey includes a few demographic questions to help us learn more about who we serve, followed by questions tailored to your relationship with EFGC. You’ll only be asked questions relevant to your experience. It takes less than 10 minutes to complete.
